Module 2.1 How to Build Forms

In this module, you will be introduced to form building. Forms are important in Zengine, they are the foundation of the platform and where your data is stored.

Objective: At the end of the module, you will understand how forms are built in Zengine.


Your Actions:

Thank you for taking the time to watch the video. Now we want you to follow these steps to build your first two forms in your training workspace.

1. Select the Settings and Tools gear icon at the top right corner

2. Select the Forms option on the top row.

3. At the top left corner select the green Add Form Button 


4. A drop down option will appear, select the first option “Create a form via the Form Builder” 

5. Next, a Pop-up will appear asking you to name the form. In the text box name the form “PROFILE”

6. The form editor will open. We are going to create a profile form to collect identifying information from our applicants/constituents. The image below is a copy of the form we are going to make. We will also describe the fields you need to use to create this form

7. First, there will be a text field on your canvas titled, “Record Title”. Click on the word “Record” and change the entire phrase to read “First Name”.

8. Open the field options for the text by select the “Edit” button at the right side of the text field

9. Select the required checkbox (This option makes the field a mandatory field for constituents to fill in.)

10. Next, drag a text field onto the canvas below the “First Name” text field.

11. Label the text field “Last Name”

12. Open the field options for the text by select the “Edit” button at the right side of the text field

13. Select the Required checkbox

14. Drag another text field onto the canvas below “Last Name”

15. Label the text field Email Address

16. For the Email Text field, select the Required checkbox.

17. Next, Select the Advanced options link at the bottom of the text field

18. Scroll to the bottom of the field to the Validation settings and select “Email” This will force applicants to type in a valid email address.

19. Drag in a new text field and place it under the Email field

20. Label the field Phone Number

21. Open the field options for the field and select the Required checkbox

22. Next, Select the Advanced options link at the bottom of the text field

23. Scroll to the bottom of the field to the Validation settings and select “Number” This will force applicants to type in numbers only.

24. Drag in a new text field and place it under the the Phone Number field and Label the field Address

25. Make the field a required field

26. Drag a new field and place it under the Address and label the new field “City”

27. Make the City field required.

28. Drag in a State Field from the palette. The State field is a pre-populated dropdown menu containing all US states. 

29. Make the State field required.

30. Finally drag in a text field, make it required, then open the advanced options and set the field to Zip Code in the Validation section.

31. Select the blue, Save button at the bottom right of the page.

Congratulations! You have successfully set up your first form!


Now we need to make an Application form.

1. At the top of the page select the View forms button.

2. At the top left corner select the green Add Form Button 

3. A drop down option will appear, select the first option “Create a form via the Form Builder”       

4. Next, a Pop-up will appear asking you to name the form. In the text box name the form “APPLICATION”

5. When the form editor opens, scroll to the bottom of the palette and drag in a header field above the first field. 

6. Label the header field General Information

7. Next, change the label of the Record Title field to “What is the name of your project?”

8. Make the text field required.

9. Drag in a new text box field and label the field “Why should your project be considered?”

10. Open the field options and set the field to required.

11. Open the advanced options for the field and scroll down to the bottom of the options. Under the Validation Options, set the max amount of words to 100

12. Next, drag a date field under the the last field and label the field “When will your project start”

13. Open the date field options and set the field to required

14. Under the date field, drag in a header field and label the header “Key Performance Indicators”

15. Next, drag in a text box field and label it “What impact will your project have in the community you serve?”

16. Open the field options, set the field to required and set the word count to 500 words (If you don’t remember how to set a word count see step 11).

17. Next drag in a checkbox field under the last field and label this field “Will your project impact (select all that apply).”

18. Open the field options. You will see two columns, one titled “Value” and the other titled “Label”.

    1. In the first row, type “children” for both the Value and Label.
    2. In the second row, type “women” for both the Value and Label
    3. In the third row, type “men” for both columns
    4. Finally delete the last row by selecting the trash can on that row.

19. At the bottom of the column is the required checkbox. Select it to make the field required.

20. Next, drag in another checkbox field below the last one. Label the new checkbox field “Which groups will be impacted (select all that apply)”

21. Open the field options. 

    1. In the first row, type “Indian/Alaskan” for both the Value and Label.
    2. In the second row, type “Asian” for both the Value and Label
    3. In the third row, type “Black” for both columns
    4. In the fourth row, type “Hispanic/Latino” for both columns
    5. You will need to add in 3 more rows. Select the blue “Add Option” button 3 times.
    6. In the fifth row, type “Hawaiian/Pacific Islander” for both columns
    7. In the sixth row, type “White” for both columns
    8. In the last row, type “Unknown” for both columns

22. At the bottom of the columns is the required checkbox. Select it to make the field required.

23. Drag in another checkbox field under the last field and label it “What ages will be impacted (select all that apply)”

24. Open the field options. 

    1. In the first row, type “1-18” for both the Value and Label.
    2. In the second row, type “18-55” for both the Value and Label
    3. In the third row, type “55-80” for both columns
    4. Finally delete the last row by selecting the trash can on that row.

25. At the bottom of the column is the required checkbox. Select it to make the field required.

26. Next, drag a header field below the last field and label it Financial Information

27. Now drag in a number field and label it “What is the total budget for your project”

28. Open the number field options:

    1. Set the field to required
    2. Check the “Set number to currency” box
    3. In the currency symbol dropdown menu, select $ - US dollars(USD)

29. Now, drag another number field under the last field and label it “What is the amount you would like funded?”

30. Open the number field options:

    1. Set the field to required
    2. Check the “Set number to currency” box
    3. In the currency symbol dropdown menu, select $ - US dollars(USD)

31. Open the number field advanced options

    1. In the help text field box type in “Maximum request is $10,000”
    2. In the Validation Options section find the “Maximum Value” text field and enter “10000”

32. Finally select the blue “Save button” at the bottom left corner.
